There are several health forms needed to be submitted throughout a child's elementary years. Each student at every grade level is required by the Iowa Immunization Law to have a Certificate of Immunization form or alternatively an exemption form on file with the school. A child without a valid form may be withheld from school until this documentation is received.

Four-year-old preschool students are required by a statewide grant to have documentation of a physical examination. For all other grades, a physical examination form is only recommended.

Students entering Kindergarten will need a Certificate of Dental Screening form as well as a Certificate of Vision Screening form.

Children entering the 3rd grade will require a Certificate of Vision screening form only.

The dental screening can be performed by a licensed dentist, dental hygienist, physician, physician assistant, RN, or ARNP.  A screening anytime after the age of 3 years old will meet the dental screening requirement.

A vision screening no earlier than 1 year prior and no later than six months after enrollment of Kindergarten and 3rd grade will meet the requirement. An ophthalmologist, pediatrician, physician, or professional licensed for this test can perform the screening.
